Updates to BMJ Best Practice topics

 The following important updates have been made to BMJ Best Practice topics. NICE recommends teplizumab to delay progression to stage 3 type 1 diabetes Topic: Type 1 diabetes , Section: Management: emerging The National Institute for Health and Care Excellence (NICE) in the UK now recommends teplizumab as an option for delaying the onset of stage 3 type 1 diabetes in adults and children aged 8 years and over with stage 2 type 1 diabetes. Previously, there were no treatments available to delay progression to stage 3 disease, and management was limited to monitoring, education, and psychosocial support. European guidance lowers the bar for starting antiviral therapy in chronic HBV Topic: Hepatitis B , Section: Management: approach The European Association for the Study of the Liver (EASL) has published its updated clinical practice guideline on the management of hepatitis B virus (HBV) infection. NIHR Research Updates (Sept 2026)

 Latest highlights from NIHR. Exercise mitigates prostate cancer treatment side effects We have funded a landmark NHS trial which has shown that a targeted lifestyle programme can reduce the side effects of hormone therapy for men with prostate cancer. It found that supervised exercise, dietary advice and behavioural support significantly improve quality of life for patients. First patient in live AI assisted sight-saving brain surgery A neurosurgeon was supported by artificial intelligence (AI) in real time during live surgery for the first time, helping to protect the sight of a man having a brain tumour removed. We part-funded and supported the clinical trial which used AI technology . Rhys Hibbert, 48, from Bedfordshire, was the first patient to have surgery in this way.

Keywords: Orthopaedic and trauma surgery; Plastic and reconstructive surgery. Case presentation A right-hand-dominant man in his 60s presented with several years of progressive flexion deformity of the right little finger. The proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints were fixed at 90°. Examination revealed a firm cord-like structure along the ulnar aspect of the digit, without palmar nodules, other digit involvement or triggering. Digital nerve function and perfusion were normal.
